INITIAL SCHOOL VISIT.
Please supply a possible date for you and your child to visit the school. This is necessary for assessment, collection of materials and to meet the class teacher.

SUPERVISION ACKNOWLEDGEMENT:
I/we certify that the conditions stated can be met. I/we realise that the home supervisor must work with the class teacher to teach the child for whom enrolment is sought. I/we realise that the class teacher will expect a weekly report, including either taped or written messages.

CONDITIONS OF ENROLMENT.
GENERAL.
(a) One set of lessons is to be completed and returned weekly*
* Overseas Enrolments: All work is to be returned weekly by airmail as soon as possible after a week's work has been completed. Supplementary readers and cassette tapes must also be returned by airmail.

(b) The daily undertaking of lessons (Monday to Friday) except where illness or other urgent matters intervene. Absence notes required as per "Public school"

(c) Parents will contact the school where periods of illness etc. occur.

(d) The observance of N.S.W. state school and public holidays (i.e. no lessons to be undertaken during those periods).

(e) Provision of an adult English-speaking supervisor.

(f) A working cassette recorder is essential to listen to the teacher's weekly message and record a supervisor's and child's message. You will also need a simple calculator with a Constant function on it.

(g) Students are encouraged to visit other schools for social contact. However, this must not be more than 1 day a fortnight. The Student cannot be enrolled at that school. There is no obligation on a local school to accept Distance Education students.

(h) Travelling Enrolments: If possible an itinerary is to be provided 5 weeks in advance during all stages of enrolment. Alternatively ring/fax/e-mail the school weekly with the next forwarding address. Using a third party for the forwarding of lessons is NOT PERMITTED. Note: Forwarding of missed mail is at your own expense.

(i) Minimum period of enrolment is six months. (j) If you discontinue your enrolment or any changes in circumstances occur, you must notify the school immediately.
